Patient Rights and Responsibilities

This document describes your rights as a patient of Pelerin Medical Services, PLLC (the "Practice") and the responsibilities you accept when receiving care through the Novus MD platform.

Your Rights as a Patient

  • Right to Respectful Care — considerate, respectful care without discrimination on the basis of race, color, national origin, sex, age, disability, religion, or any other characteristic protected by applicable law.
  • Right to Informed Consent — clear information about your diagnosis, proposed treatment, alternatives, and risks/benefits in language you can understand, and the right to ask questions before consenting.
  • Right to Refuse Treatment — you may refuse any recommended treatment or medication and be informed of the potential consequences, without affecting your right to respectful care or to seek services elsewhere.
  • Right to Clinical Independence — your clinician's decisions are made independently, based solely on your clinical needs. Novus MD LLC has no authority over clinical decisions.
  • Right to Privacy and Confidentiality — your health information is handled per the Practice's Notice of Privacy Practices and applicable law, including HIPAA.
  • Right to Access Your Records — you may inspect, obtain a copy of, and request amendment to your medical records via the Privacy Officer.
  • Right to Know Your Care Team — you may know the name, credentials, and role of your clinician, and may request a different clinician subject to availability.
  • Right to Continuity of Care — you have the right to information and support necessary to continue your care if you leave the Practice or the Practice is unable to continue serving you.
  • Right to File a Complaint — without fear of retaliation, to the Privacy Officer, your state medical licensing board, or the U.S. Department of Health and Human Services Office for Civil Rights at www.hhs.gov/ocr.
  • Right to Emergency Referral — if your situation requires care beyond telehealth, you have the right to be informed and referred to appropriate in-person services.

Your Responsibilities as a Patient

  • Provide Accurate Information — complete and accurate medical history, medications, allergies, and other relevant information.
  • Complete Required Laboratory Testing — baseline testing before hormone therapy begins, and ongoing monitoring at intervals not exceeding six months. Treatment cannot begin or continue without required lab results.
  • Attend Scheduled Consultations — or provide advance notice to reschedule.
  • Follow Your Treatment Plan — including medication instructions and dosing; discuss concerns with your clinician rather than making changes independently.
  • Notify Us of Changes — in your health status, new medications, diagnoses, or adverse reactions.
  • Use the Platform Appropriately — for your own care only; do not share account access or attempt to obtain prescriptions on behalf of others.
  • Seek Emergency Care When Needed — telehealth is not a substitute for emergency care. Call 911 or go to the nearest emergency room in an emergency.
  • Honor Financial Commitments — maintain a valid payment method and honor your Membership Agreement, including the commitment term disclosed at sign-up.
