Prescription Policy

This Prescription Policy describes what clinical services and medications the Practice offers, the standards that govern prescribing decisions, and what patients should understand before beginning care.

1. Scope of Clinical Services

The Practice provides telehealth-based hormone optimization services, including Testosterone Replacement Therapy (TRT), for both men and women. The Practice may also prescribe certain peptide therapies where clinically appropriate, subject to the same individualized clinical evaluation as any other prescription. Weight management / GLP-1 services are planned as a future offering and are not currently active.

The specific services available to you depend on your membership tier, your state of residence, and your clinician's assessment of your individual clinical situation.

2. What We Prescribe

The Practice prescribes compounded testosterone and related hormone optimization medications where clinically appropriate. All medications are compounded by licensed 503A compounding pharmacies and shipped directly to your home.

Compounded medications are not FDA-approved. Compounding pharmacies prepare medications for individual patients based on a clinician's prescription. These medications are not subject to FDA approval and may differ from commercially manufactured products.

3. No Guarantee of Prescription

Purchasing a Novus MD membership does not guarantee that you will receive a prescription. All prescribing decisions are made exclusively by your licensed clinician based on:

• review of your medical history and intake information;
• mandatory baseline laboratory results;
• clinical evaluation during your telehealth consultation; and
• your clinician's independent professional judgment.

The Practice will not prescribe outside of clinically appropriate standards regardless of membership status or patient preference.

4. Mandatory Laboratory Requirements

Before any hormone therapy is initiated, you are required to complete a baseline laboratory panel. Treatment cannot begin until baseline lab results are reviewed by your clinician — there are no exceptions. Ongoing laboratory monitoring is required at intervals not exceeding six months for all patients on active hormone therapy. Failure to complete required monitoring may result in a pause or discontinuation of your prescription.

5. Controlled Substance Prescribing Standards

Testosterone is a Schedule III controlled substance under the federal Controlled Substances Act. The Practice prescribes testosterone in compliance with DEA registration requirements, Electronic Prescribing of Controlled Substances (EPCS) standards, applicable state controlled substance registration requirements, and DEA telehealth prescribing flexibilities currently in effect through December 31, 2026.

No prescription quotas. The Practice does not establish quotas, targets, or expectations regarding the number or type of prescriptions issued.

6. Off-Label Prescribing

Testosterone therapy for women is considered off-label use — it has not been specifically approved by the FDA for this indication. Off-label prescribing is legal and common in clinical practice. Your clinician will explain the evidence base, potential benefits, and risks during your consultation and obtain your informed consent before initiating treatment.

7. State Availability

The Practice's prescribing services are available only in states where the Practice holds an active license and where applicable telehealth prescribing laws permit delivery of hormone therapy via telemedicine.

Services are currently available in: Michigan and Massachusetts.

8. DEA Telehealth Cliff

The Practice's current remote prescribing model operates under DEA telehealth flexibilities extended through December 31, 2026. If these flexibilities are not renewed, the Practice may require an in-person evaluation prior to prescribing. Active patients will be notified well in advance of any change.

9. Discontinuation of Prescriptions

Your clinician may discontinue or modify your prescription at any time based on clinical judgment, including changes in your lab results, health status, or safety concerns. The Practice will notify you of any such change and explain the clinical basis.
